Abbott Diabetes Care Retail Medicare Rebate Program
1. How do I qualify to receive the rebate?
 
  • Pharmacies must complete the online enrollment and agree to the terms and conditions to receive the rebate.
  • Pharmacies must be approved for the Abbott Diabetes Care Retail Medicare Rebate Program.
  • Pharmacies must purchase approved FreeStyle and Precision products from Abbott Diabetes Care approved wholesalers (see below).
  • Pharmacies must submit claims through one of the following Medicare Claims Adjudicators (OmniSYS, Freedom Data Services (EZ-DME), or Emdeon (eRx Network/Allwin)).
2. How will I know when my pharmacy is enrolled for the Abbott Diabetes Care rebate?
  • You will receive an email within the next two weeks confirming that your pharmacy has been approved to receive the rebate
  • If you would like to check the status of your application, please go to the Enroll Now page.
3. How will I know if I am currently enrolled in the Abbott Diabetes Care (ADC) Retail Medicare Program?
  • To confirm that you are currently enrolled in the ADC Retail Medicare Program, simply enter your DEA #, NABP/NCPDP# and state on the registration page.
4. How long will it take to receive my rebate check?
  • Rebates will be paid on a monthly basis following Abbott Diabetes Care's receipt of appropriate data from approved Adjudicators.
